Provider Spotlight
Prairie Clinic LLC opened its doors to primary care patients on April 1st, 2024, owned by a Family Nurse Practitioner (FNP) who works in collaboration with her team to provide the highest quality primary care to every patient they encounter. They apply evidence-based knowledge along with clinical experience to determine a patient-centered plan of care to move them along on the continuum of health in their life journey. They hold the individual’s right to autonomy as an utmost priority in their day-to-day practice.
Tennille Rau, DNP, FNP and Practice Owner at Prairie Clinic LLC, pictured left below next to Jenna Homsher, NP. shared the following testimonial about her experience with the Colorado Medicaid eConsult Platform:
“We are new to the RAE and Northeast Health Partners this year. That is where we were introduced to the eConsult Platform. We completed the onboarding and training with relative ease and little interruption from our practice. The platform is easy to navigate.
It was nice to be able to submit a case and have expert review and input in 1-2 business days for patients who needed specialty care and were waiting to get into the specialist. Both Jenna and I learned a lot from the responses from the various specialties that we did a consultation with.
We do recommend this platform to other Primary Care Providers. In our busy primary care settings this is an easy tool that is at our fingertips when we need it. It can improve patients’ care, providing timely expert interventions while the patient is waiting to see a specialist for their problem. This has increased access to specialty care for our patients.”
-Tennille Rau, NP and Practice Owner at Prairie Clinic LLC
Prior to opening Prairie Clinic LLC, Tennille and many of her staff worked at a FQHC or Rural Health Clinics with a focus on integrated care. They wanted to have more power to improve provider and staff satisfaction, so they created their ideal primary care office. It is a long-term relationship in which they desire to give patients continuity, comfort, and familiarity leading to trust and ultimately better health over their lifespan. They see patients from Baca, Prowers and Kiowa counties, offering in person and telehealth visits.
Crafting a Strong Clinical Question for eConsults
Submitting a clear and well-structured clinical question is key to getting the most out of your eConsult. These tips help Referring Providers and Staff Submitters create focused submissions so Specialist Reviewers can provide timely and actionable recommendations.
Start with the essentials:
Include patient demographics, relevant history and comorbidities, current medications with dosages, the chief complaint, prior tests or treatments, and a concise clinical question that states exactly what guidance you need.
Add supporting documentation:
Progress notes, labs, imaging, and photos (when appropriate) help specialists complete their review. Check the Supporting Documentation Guide in the User Guides and Info section of the Colorado Medicaid eConsult platform for helpful items to include by Specialty.
Use templates for consistency:
Standardized templates in your EHR and/or eConsult platform to ensure all key details are captured.
Don’t Forget:
The Referring Provider will be notified when the Specialist Reviewer responds to the eConsult. They can then log in to review the guidance and sign off on the eConsult.
The eConsult summary is available for any user in the clinic to download and add to the patient record in your EHR.
All related resources, including the Staff Submitter Tip Sheet and the Supporting Documentation Guide, can be found in the User Guides and Info section of the Colorado Medicaid eConsult platform.
Reminder: Staff and Specialists Can Submit eConsults!
Trained staff can now initiate and submit eConsults on behalf of Referring Providers, helping streamline workflows and reduce turnaround times for Medicaid members.
Specialists can also submit eConsults, leading to smarter collaboration, equitable access, and delivering better outcomes for Medicaid members.
